• Monthly Subscription Fee of $100 (billed on a monthly basis via CREDIT CARD or paid in full up front at a 15% discount i.e. $995.00)
• Per Employee Fee of $25 for managing the compliance effort for a particular employee.* |

*Note: For both models this per employee fee is charged only for those employees that are subject to the HIRD form requirement i.e. those employees
not eligible for the employer Healthcare or those who are
eligible but have waived the employer plan.
