The Cost for a *ComplyPRO eXpress* software only subscription:
	There are 2 components to the *ComplyPRO eXpress* software only subscription cost. Software Only	• Subscription Fee of $20 Monthly (billed on a monthly basis via CREDIT CARD or paid in full up front at a discount ie. $195.00) plus • Per Employee Fee of $25* for managing the compliance effort for a particular employee.* This is a per employee fee, based on a sliding scale depending on the number of employees receiving HIRD form requests.

The Cost for a *ComplyPRO eXpress* subscription plus Concierge Service:
	There are 2 components to the *ComplyPRO eXpress* subscription plus Concierge level service cost. Software plus Concierge Service	• Subscription Fee of $50 Monthly (billed on a monthly basis via CREDIT CARD or paid in full up front at a discount ie. $490.00) plus • Per Employee Fee of $25* for managing the compliance effort for a particular employee. This is a per employee fee, based on a sliding scale depending on the number of employees receiving HIRD form requests.

*Note: For both models this per employee fee is charged only for those employees that are subject to the HIRD form requirement i.e. those employees not eligible for the employer Healthcare or those who are eligible but have waived the employer plan. Below is the way volume of services a user consumes affects the cost.

An Example: If a company had 30 employees who were eligible for HIRD forms the cost would be $750 ( 30 X $25). If a Company had 125 employees who received HIRD Kits the cost would be $3,000 per year ( First 100 X $25, the 25 X $20).

*	For first 100 employees per year	$25 per employee
	From 101 to 200 employees per year	$20 per employee
	From 201 to 500 employees per year	$15 per employee
	Over 500 employees per year	$10 per employee
